When must carriers pay, reduce, or deny a medical bill?

Carriers must pay, reduce, or deny a bill within 30 days of receiving it. If they deny or reduce the bill, they must explain the reason. After a denial, the employer or carrier has an additional 15 days to request Utilization Review if they believe the treatment was not reasonable or necessary.
