When can an employer or carrier request UR?

A carrier or employer can request UR after they deny or reduce a medical bill because they believe the treatment is not reasonable or necessary. UR must be requested within 15 days after the denial. Employers should work closely with their carrier to monitor treatment and decide when UR is needed.
