What documentation is accepted as proof of Workers’ Compensation insurance coverage?

The Department of Labor may require employers to file evidence of coverage, such as a certificate of insurance, policy declarations page, or proof of approved self-insurance. When notified that a policy has been canceled or lapsed, an employer generally has 14 days to provide proof of insurance or show that the business has closed or no longer has employees.
