What deadlines apply to UR requests?

Carriers must review a medical bill within 30 days of receiving it. If they deny it, they then have an additional 15 days to submit a UR request. Missing the deadline may prevent the carrier from challenging the treatment. These timelines help keep medical decisions moving quickly.
